Support at Home Guide

To help you understand more about the Government's Support At Home, we have compiled some of the key details on the program and how you can get access to register.

Changes to Support At Home

Previously known as Home Care Packages the government has released a new initiative called Support At Home, the purpose and intention of the program is to allow you to have all the services you require which can assist you to remain living in the your home for longer.

What Services are available?

There are a range of services available. Services are available through 3 categories each with their own service type, these are:

Clinical Supports

  • Nursing Care
  • Allied Health
  • Nutrition
  • Care Management
  • Restorative Care Management

 

Independence

  • Personal Care
  • Social Support
  • Therapeutic Services
  • Respite
  • Transport
  • Assistive Technology and Home Modifications

 

Everyday Living

  • Domestic Assistance
  • Home Maintenance and Repairs
  • Meals

What Package Levels are there?

There are 8 levels of packages which are available through Support At Home and the level you are allocated will be determined through an integrative assessment tool performed by My Aged Care.

How do I Choose a service?

When commencing with Support At Home you will be allocated a care partner through Gilgandra Lifestyles who will meet with you to access your levels of care needs and how best to develop a care plan with you that will make best use of your allocated budget.

We will continue to regularly work with you to ensure you are receiving all the care required to meet your needs and continue to update this with you where appropriate. 

How much does it cost?

If you access government-funded aged care services through the Support at Home program, your contributions will be based on your income and assets, and they will vary by the amount and type of services you receive.

When you receive Support at Home services, you have a budget to pay for the services you need. Your budget depends on your assessed care needs and includes government funding and your contributions.

It’s a good idea to complete your income and assets assessment before you choose your provider. Knowing how budgets work can also help you when comparing, choosing, and starting to receive services from your chosen provider.

At Gilgandra Lifestyles you can expect to pay a basic daily care fee (this rate is determined by the government) which will mean you are out of pocket 25% of this rate to have your Support At Home Service. You can expect to pay 0% of the income tested fee.

If you are a new Support At Home client based on the information provided below is a table that outlines the cost of what you are required to contribute, the rest of the cost will be funded by the government through your package.

Can I cancel the service if I no longer require it?

You may exit from your service agreement at any time if you no longer require the service. If you wish to cancel, please contact your care partner so they can arrange the cancellation of the service.

How can I find out more?

To determine eligibility, you need to contact My Aged Care who will do an assessment. Each client is individually assessed to ensure all of their care needs can be adequately met. Once eligibility has been determined a package can then be tailored to meet your identified care needs.

If you are assessed as eligible for a home care package you will:

  • receive a letter of approval from My Aged Care that sets out the level of home care package you are approved to receive
  • be placed in a national priority queue for home care packages and will be contacted when a suitable package becomes available for you.
