Clinical Support

Our clinical services focus on your health, safety, and wellbeing, supporting you to manage health needs at home.
Services may include:

  • Nursing care and clinical support
  • Medication management
  • Wound care and continence support
  • Health monitoring and chronic condition support
  • Allied health services such as physiotherapy, occupational therapy, and podiatry

 

Independence

These services are designed to help you maintain your independence, ensuring you can manage day-to-day activities safely and confidently.
Services may include:

  • Personal care (showering, dressing, grooming, mobility assistance)
  • Assistance with medications and daily routines
  • Use of assistive technology or mobility aids
  • Support with appointments and transport

 

Everyday Living

Our everyday living services focus on keeping your home safe, comfortable, and welcoming, while supporting your connection to the community.
Services may include:

  • Domestic support (cleaning, laundry, meal preparation, shopping)
  • Gardening and light maintenance
  • Social support and wellbeing programs
  • Assistance to participate in community and social activities

 

Assistive Technology (AT) & Home Modifications

We understand that sometimes, the right equipment or a small change at home can make a big difference in your safety, comfort, and independence.
Through SAH, you may be eligible for:

  • Installation of ramps, grab rails, and bathroom aids
  • Modifications to improve access and mobility around the home
  • Assistive technology such as shower chairs, walking aids, sensor lighting, or other safety devices
  • Professional advice and assessments to ensure the right fit for your needs
